Do you only offer Host Homes?
No. We also offer Supported Community Living (hourly or 24/7 in‑home support) and Habilitation Services (education, skills, and community integration).
Who is eligible for each program?
Eligibility depends on support needs and funding. We’ll help determine the best fit during intake.
How quickly can services start?
Timing varies by program and availability. After referral, we move through fit, matching, and onboarding as quickly as possible.
What do you look for in a Host?

How do I make a referral?
